Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with activities of daily living
• Take and record vital signs accurately and report changes to the nursing team
• Support safe transfers and mobility, using proper body mechanics and equipment
• Assist with personal care tasks such as bathing, dressing, and feeding with dignity
• Document resident care and observations clearly and promptly in the chart
• Communicate effectively with nurses, therapists, residents, and families
• Contribute to a positive rehabilitation-focused environment that encourages recovery
• Follow infection control policies and facility safety protocols at all times

Qualifications

• Certified Nursing Assistant (CNA) credential required; NY certification preferred
• Current CPR/BLS certification preferred or willing to obtain
• Ability to pass background checks and required health screenings
• Comfortable working in a long-term care or rehabilitation setting
• Strong communication skills and a compassionate patient-first attitude
• Reliable, punctual, and able to work independently when needed

Education

• High school diploma or GED
• Completion of an accredited CNA training program

Knowledge & Skills

• Hands-on patient care and assistance with ADLs
• Basic vital signs measurement and documentation
• Safe transfer and positioning techniques
• Familiarity with long-term care and rehab routines
• Effective communication and teamwork with interdisciplinary staff
• Basic electronic charting or paper charting skills
