Roles & Responsibilities

• Provide direct patient care in a skilled nursing facility, ensuring comfort and safety
• Administer medications, treatments, and wound care per physician orders
• Monitor vital signs and assess patient conditions, reporting changes promptly
• Collaborate with interdisciplinary team to develop and implement care plans
• Educate patients and families on health management and discharge planning
• Maintain accurate documentation and adhere to facility policies and regulations

Qualifications

• Valid LPN or RN license in the state of Alabama (or compact state license eligible)
• Current BLS certification (American Heart Association)
• Minimum of 1 year of recent experience in skilled nursing or long-term care
• COVID-19 vaccination or approved medical/religious exemption per facility policy
• Ability to pass a background check and drug screening
• Strong communication and teamwork skills

Knowledge & Skills

• Proficient in patient assessment, medication administration, and wound care
• Knowledge of electronic health records (EHR) systems
• Excellent empathy and patient advocacy skills
• Ability to adapt quickly to new environments and protocols
• Time management and organizational skills to handle multiple priorities
