Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, weekends every 3-4 weeks if at all
Hours per Week: 40
Candidate Pay Rate: $50 per hour

Roles & Responsibilities
- Complete comprehensive Social Work assessments to understand each patient's unique needs
- Conduct Mental Health assessments and provide vital support during critical moments
- Coordinate thoughtful discharge planning to ensure smooth transitions of care
- Assist with Title 36 petitions and connect patients with essential community resources
- Support individuals facing homelessness, substance abuse, and complex social determinants of health
- Provide maternal child assessments and collaborate with the care team across inpatient, ER, ICU, and Family Birthing Center settings
- Report to DCS and APS as needed to advocate for patient safety
Qualifications
- Active LMSW or LCSW license in Arizona
- Comfortable working with Cerner EMR
- Experience with inpatient, emergency, and maternal-child populations is a plus
- Strong assessment and care coordination skills
Knowledge & Skills
- Social Work assessments and Mental Health evaluations
- Discharge planning and care coordination
- Navigating complex situations involving homelessness and substance abuse
- Understanding of SDOH and community resources
- Clear documentation and collaboration within a multidisciplinary team
