Location: Louisville, Kentucky

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide comprehensive physical therapy services in patients' homes
- Conduct start of care assessments and evaluations
- Develop and implement personalized treatment plans
- Perform recertifications and routine visits for ongoing care
- Support patients recovering from CHF, orthopedic conditions, and pneumonia
- Collaborate with clinical managers and educators for optimal patient outcomes
Qualifications
- Active Physical Therapist license in Kentucky
- BLS certification required
- Home health experience preferred but not required
- Valid driver's license and reliable transportation
- Strong communication and documentation skills
Knowledge & Skills
- Home health physical therapy expertise
- Patient assessment and care planning
- Electronic documentation and tablet proficiency
- Compassionate bedside manner
- Time management and independent work skills
