Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in our SNF/Sub Acute unit
• Assess, plan, and implement individualized care plans based on patient needs
• Administer medications and treatments accurately and safely
• Monitor patient conditions and document progress using the Cerner charting system
• Collaborate with physicians, therapists, and the care team to ensure optimal outcomes
• Educate patients and families on health management and discharge planning

Qualifications

• Active California RN license
• BLS Certification (required)
• Minimum of 2 years of experience in SNF, sub-acute, or long-term care setting
• Familiarity with Cerner charting system is preferred
• BSN not required - ADN or diploma graduates welcome

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Proficient in medication administration and IV therapy
• Experience with electronic medical records (Cerner preferred)
• Excellent communication and interpersonal skills
• Ability to work in a fast-paced post-acute environment
• Team player committed to patient-centered care
