Contract Type: Contract

Shift Type: Rotating shifts with weekends required approximately every 4 weeks and all minor holidays plus 2 major holidays worked

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Provide compassionate end-of-life care to hospice patients in their homes and facilities
Manage a caseload of 12-14 patients with 4 visits or 2 start-of-care assessments daily
Build meaningful relationships with patients and families during their most vulnerable times
Document care using Home Care Home Base EMR system
Collaborate with interdisciplinary team to ensure comprehensive patient support
Travel within the greater Sacramento area to deliver personalized care

Qualifications

Active Registered Nurse license in California
Minimum 1 year of hospice experience required
Current BLS certification
Strong communication and compassionate bedside manner
Ability to work independently while providing excellent patient care

Knowledge & Skills

Hospice care expertise and end-of-life support
Home care and facility-based patient management
Electronic medical records documentation (Home Care Home Base)
Patient assessment and care planning skills
Family education and emotional support abilities
