Roles & Responsibilities

• Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician's orders
• Serve as Case Manager for patients when this role is the lead discipline
• Conduct comprehensive OASIS assessments and develop care plans
• Coordinate care with interdisciplinary team members and community resources
• Educate patients and families on health promotion, disease management, and medication regimens
• Document patient progress accurately and timely in electronic medical records

Qualifications

• Active RN license in Vermont (must be currently active)
• Minimum 1 year of home health experience
• OASIS experience is mandatory
• BLS/BCLS certification required
• Homecare Home Base experience preferred
• Experience as a home care traveler with OASIS proficiency preferred

Knowledge & Skills

• Expertise in OASIS documentation and assessment
• Strong clinical assessment and critical thinking skills
• Knowledge of home health regulations and Medicare guidelines
• Excellent communication and interpersonal skills
• Ability to work independently and manage a caseload
• Proficiency in electronic health records (Homecare Home Base preferred)
