Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Deliver compassionate physical therapy care to patients in their homes across greater Sacramento county
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Help patients regain mobility and independence with personalized treatment plans
Collaborate with interdisciplinary teams to ensure the best outcomes for every patient
Document visits accurately and manage mileage records per GSA guidelines

Qualifications

Active Physical Therapy Assistant license in California
Prior home health experience preferred
New grads considered only in exceptional cases
Strong communication and time-management skills
Ability to work independently in various home settings

Knowledge & Skills

Home health care best practices
Patient assessment and treatment planning
Cardio, pulmonary, orthopedic, neurological, and geriatric care
Accurate documentation and mileage tracking
Building rapport with patients and families
