Roles & Responsibilities

• Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans
• Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence
• Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning
• Educate patients and caregivers on safe mobility techniques, equipment use, and home-exercise programs
• Supervise and coordinate care with Physical Therapist Assistants (PTAs)

Qualifications

• Licensed Physical Therapist in North Dakota (must be currently active)
• Current driver's license and reliable car for travel to patient homes
• At least 2 years of previous experience as a Physical Therapist preferred
• BLS/BCLS certification required

Knowledge & Skills

• Skilled in physical therapy assessment and evidence-based treatment planning
• Strong patient education and caregiver training abilities
• Excellent collaboration and communication for interdisciplinary teamwork
• Ability to work independently in home health and outpatient settings
