Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover from joint replacements and manage chronic conditions like CHF and COPD
Complete approximately 5 meaningful visits each day while building genuine connections
Document care using HomeCare HomeBase EMR on your provided tablet
Collaborate with the intake RN and manager to coordinate excellent patient outcomes

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health background
Comfortable working independently in home settings

Knowledge & Skills

HomeCare HomeBase EMR system
Home Health PT specialty experience
Strong assessment and clinical decision-making abilities
Excellent communication and patient education skills
Ability to manage a territory with approximately 40 miles of daily driving
