Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in the comfort of their homes
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and other orthopedic procedures
Support geriatric patients managing multiple chronic conditions like CHF and COPD
Document visits accurately using HomeCare HomeBase EMR on your provided tablet
Collaborate with the intake RN and manager to coordinate seamless patient care

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health background
Compassionate approach to caring for orthopedic and geriatric patients

Knowledge & Skills

Physical Therapy expertise in home health settings
Comfortable with EMR documentation using HomeCare HomeBase
Strong assessment and care planning skills for complex patients
Ability to manage a caseload of approximately 5 visits per day
Reliable transportation for covering service areas across Tehama County
