Roles & Responsibilities

Conduct physical therapy assessments and treatments for patients in their homes
Develop and implement individualized treatment plans
Collaborate with nursing staff and other healthcare professionals
Perform SOC visits, especially with ortho patients when nursing is not ordered
Utilize EMR system (HCHB - HomeCare HomeBase) on a provided tablet

Qualifications

Physical Therapy degree and licensure
Home Health experience (will consider travelers with less than 2 years of experience)
BLS certification
Experience with EMR systems (HCHB - HomeCare HomeBase)

Physical therapy skills
Home health experience
BLS certification
EMR system proficiency (HCHB - HomeCare HomeBase)
