Location: Wilkes Barre, PA

Start Date: 05-09-2026

Contract Type: Per-Diem

Shift Type: 8-hour shift (total hours: 8). Specific start/end times and scheduled days to be confirmed with the facility.

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care, monitoring vital signs and reporting changes
Administer medications and treatments safely and accurately
Collaborate with nurses and interdisciplinary team to implement care plans
Document patient status, interventions, and responses clearly in the medical record
Assist with activities of daily living while promoting patient dignity and comfort
Support admissions, discharges, and transfers to ensure smooth patient transitions
Communicate effectively with patients and families to educate and provide reassurance

Qualifications

Active LPN license in Pennsylvania required
Current BLS/CPR certification required or ability to obtain promptly
Comfortable with medication administration and basic wound care
Strong communication skills and a team-oriented attitude
Previous LPN experience in a long-term care or similar setting preferred

Education

Graduate of an accredited LPN / Practical Nursing program

Knowledge & Skills

Knowledge of basic nursing procedures and infection control practices
Proficient in vital signs assessment and patient observation
Familiarity with electronic medical records and accurate charting
Solid clinical judgment and ability to escalate concerns appropriately
Ability to work efficiently in a fast-paced care environment
