Roles & Responsibilities
- Provide compassionate direct patient care and conduct routine assessments
- Administer medications and treatments safely and accurately
- Document patient status, care delivered, and any changes in condition
- Collaborate with nurses, therapists, and care team members to coordinate care
- Communicate clearly and kindly with patients and families about care plans
- Support infection control and safety protocols to maintain a healthy environment
Qualifications
- Current LPN license in Pennsylvania (or compact/state-appropriate licensure if applicable)
- Active Basic Life Support (BLS) certification or willingness to obtain
- Comfortable working 8-hour shifts and delivering hands-on resident care
- Strong communication skills and ability to work as part of a multidisciplinary team
- Proof of required immunizations and background checks as facility policy requires
Education
- LPN diploma or vocational nursing certificate
- High school diploma or equivalent
Knowledge & Skills
- Medication administration and basic nursing procedures
- Patient assessment and vital sign interpretation
- Skilled charting and documentation practices
- Basic wound care and infection prevention techniques
- Ability to use electronic health record systems
- Compassionate bedside manner and strong teamwork abilities
