Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and on schedule
Perform routine assessments and report changes in condition to the care team
Document care thoroughly and maintain accurate resident charts
Collaborate with nurses, therapists and interdisciplinary staff to deliver coordinated care
Support resident comfort and dignity through respectful communication and care plans
Participate in shift handoffs and ensure continuity of care across teams

Qualifications

Current, active LPN license in Pennsylvania (required)
Graduation from an accredited practical nursing program
Current BLS/CPR certification (or willingness to obtain upon hire)
Comfortable with medication administration, basic assessments and charting
Strong communication skills and a team-first attitude
Previous long-term care or skilled nursing experience is a plus

Education

Completion of an accredited Practical Nursing (LPN/LVN) program
State LPN licensure required

Knowledge & Skills

Knowledge of nursing fundamentals and medication administration
Basic assessment and vital signs proficiency
Familiarity with electronic charting and documentation best practices
Ability to prioritize tasks in a fast-paced clinical setting
Strong interpersonal skills for patient, family and team interactions
