Roles & Responsibilities
- Provide compassionate direct patient care consistent with LPN scope
- Administer medications and treatments safely and accurately
- Monitor patient condition and report changes to the RN or provider
- Document assessments, interventions and patient responses in the chart
- Support activities of daily living and promote patient comfort
- Collaborate with the interdisciplinary team to deliver coordinated care
- Adhere to infection control and facility policies to ensure a safe environment
Qualifications
- Valid LPN license (Connecticut) required
- Current basic life support certification as required by facility policy
- Comfortable working in a long-term care or skilled nursing environment
- Strong communication and teamwork skills to partner with patients and staff
Education
- LPN diploma or certificate from an accredited program
Knowledge & Skills
- Skilled in basic nursing assessments and monitoring
- Medication administration and documentation
- Familiarity with long-term care or skilled nursing workflows
- Patient-centered care and strong interpersonal skills
- Basic electronic charting experience preferred
