Roles & Responsibilities

• Provide compassionate, patient-centered care to residents and patients
• Administer medications and treatments according to care plans
• Monitor and document patient vital signs, reporting any changes
• Collaborate with the nursing team to ensure positive outcomes
• Educate patients and families on care plans and procedures
• Maintain a clean, safe, and welcoming environment

Qualifications

• Active LPN license in Pennsylvania (or compact state)
• Current CPR/BLS certification
• Experience in long-term care, skilled nursing, or similar settings
• Strong clinical assessment and problem-solving skills
• Ability to build rapport with patients and colleagues

Knowledge & Skills

• Solid understanding of nursing practices and protocols
• Medication administration and documentation skills
• Familiarity with electronic health records (EHR)
• Excellent communication and interpersonal abilities
• Adaptable and calm under pressure
