Roles & Responsibilities

• Provide compassionate, patient-centered care to residents in our skilled nursing center
• Assess patient conditions and collaborate with the care team to create effective care plans
• Administer medications and treatments safely according to facility protocols
• Monitor vital signs and promptly report any changes in patient status
• Document patient care accurately and maintain confidentiality
• Educate patients and families on health management and discharge plans

Qualifications

• Active Registered Nurse (RN) license in Pennsylvania
• Current BLS certification (American Heart Association)
• Previous experience in long-term care, skilled nursing, or rehabilitation preferred
• Must bring state or federal issued identification to the facility and wear a photo badge at all times
• Ability to pass a background check and drug screening

Knowledge & Skills

• Knowledge of nursing care standards and best practices
• Strong communication and interpersonal skills
• Ability to work independently as well as part of an interdisciplinary team
• Proficiency with electronic health records (EHR) systems
• Time management and organizational skills
• Compassionate, empathetic approach to patient care
