Roles & Responsibilities

• Provide compassionate and attentive nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and on time
• Monitor and document patient conditions, including vital signs and changes in status
• Work collaboratively with RNs, physicians, and the care team to implement individualized care plans
• Communicate effectively with residents and their families to ensure comfort and understanding

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania
• Current BLS/CPR certification
• Previous experience in long-term care, skilled nursing, or rehabilitation is a plus
• Strong observation, communication, and documentation skills
• Commitment to providing high-quality, compassionate care

Knowledge & Skills

• Medication administration and documentation
• Patient assessment, vital sign monitoring, and reporting
• Electronic health records (EHR) and charting
• Effective communication with patients, families, and team members
• Ability to respond calmly and professionally in changing situations
