Location: Smethport, PA

Contract Type: Per-Diem
Shift Type: Day shift, 8.5-hour shift (specific start/end times and days not provided)

Candidate Pay Rate: $34 per hour

Roles & Responsibilities
- Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
- Administer medications and treatments accurately and document care thoroughly
- Monitor residents' conditions and communicate changes promptly to the care team
- Assist with ADLs and promote resident dignity, comfort and independence
- Collaborate with RNs, therapists and facility staff to support individualized care plans
- Participate in shift handoffs and maintain accurate, timely clinical records
Qualifications
- Current LPN license in Pennsylvania (required)
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Comfortable working in a long-term care/skilled nursing environment
- Strong communication skills and ability to work well in a team
- Current immunizations per facility policy and ability to pass background checks
- Professional, reliable, and dedicated to high-quality resident care
Education
- Graduate of an accredited LPN/Vocational Nursing program
- High school diploma or GED
Knowledge & Skills
- Clinical assessment and basic nursing interventions
- Medication administration and documentation best practices
- Familiarity with long-term care workflows and resident-centered approaches
- Good charting skills and attention to detail
- Ability to prioritize tasks and respond to changing resident needs
