Roles & Responsibilities
- Provide compassionate direct patient care with clinical competence and heart
- Administer medications and treatments according to physician orders and facility policy
- Monitor and document patient status, reporting changes promptly for timely interventions
- Collaborate with the care team to develop and implement patient-centered care plans
- Perform routine assessments, maintain accurate charts, and ensure continuity of care
- Support patients and families with education, reassurance, and clear communication
Qualifications
- Active Licensed Practical Nurse (LPN) license in Pennsylvania or compact state as required
- Current CPR/BLS certification
- Completion of an accredited practical nursing program
- Strong clinical skills with attention to detail and patient safety
- Excellent communication and teamwork abilities
- Prior long-term care or skilled nursing experience preferred
Education
- Graduate of an accredited practical nursing (LPN) program
Knowledge & Skills
- Medication administration and documentation best practices
- Basic assessment skills and vital sign monitoring
- Infection control procedures and standard precautions
- Electronic health record familiarity and accurate charting
- Time management and prioritization in a fast-paced setting
- Empathy, patient advocacy, and effective communication
