Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities

Evaluate residents and develop individualized physical therapy treatment plans to restore mobility and independence
Provide focused therapy to improve mobility, strength, balance, and functional recovery
Monitor resident progress and adjust treatment plans to achieve the best outcomes
Collaborate with physicians, therapists, and the interdisciplinary care team to coordinate resident-centered services
Maintain accurate and timely clinical documentation to support continuity of care
Educate residents and caregivers on exercise programs, mobility techniques, and fall prevention strategies

Qualifications

Active California Physical Therapist license (required)
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program (required)
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong clinical assessment and patient care skills
Excellent communication skills and the ability to work effectively within a multidisciplinary team

Education

Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

Physical Therapy principles and clinical assessment
Mobility training and rehabilitation techniques
Patient evaluation and treatment planning
Patient care best practices, including geriatric care and fall prevention
Clear documentation and clinical record keeping
Effective communication with residents, families, and care teams
