Roles & Responsibilities

• Evaluate patients' functional abilities and develop individualized occupational therapy plans
• Implement therapeutic interventions to improve daily living, fine motor skills, and independence
• Document progress clearly and timely to ensure continuity of care and measurable outcomes
• Collaborate with nurses, physical therapists, speech therapists, and the care team to optimize recovery
• Educate patients and families on adaptive strategies, home modifications, and adaptive equipment
• Discharge plan coordination to support safe transitions back to home or next level of care
• Participate in team meetings and contribute clinical insights to improve patient outcomes

Qualifications

• Licensed Occupational Therapist in Pennsylvania, or eligible to obtain PA licensure
• Current BLS/CPR certification (maintain per facility requirements)
• Experience in acute rehab, skilled nursing, or similar settings preferred
• Strong documentation skills and familiarity with electronic medical records
• Excellent communication and teamwork skills, with a patient-centered approach
• Ability to pass facility onboarding requirements (background check, health screening)

Education

• Master's degree in Occupational Therapy or OTD (entry-level clinical degree required)

Knowledge & Skills

• Skilled in functional assessments and ADL/IADL training
• Knowledge of adaptive equipment and splinting techniques
• Comfortable with mobility training and fall risk assessment
• Proficient documentation and care planning skills
• Ability to teach and coach patients and caregivers in practical strategies
