Roles & Responsibilities

• Provide compassionate rehabilitation services to residents in a skilled nursing and intermediate care facility, tailoring care to individual needs.
• Conduct thorough evaluations and reassessments to develop and adjust personalized treatment plans.
• Implement evidence-based therapeutic interventions to maximize mobility, function, and quality of life.
• Educate residents, families, and caregivers with clear guidance and support for continued progress.
• Maintain precise and timely documentation to ensure compliance with all regulatory and payer standards.
• Collaborate with the interdisciplinary team and supervise therapy assistants to deliver comprehensive care.

Qualifications

• Active Hawaii State license in Physical Therapy Assistance (or ability to obtain before start)
• Current BLS/BCLS certification
• Minimum of 2 years of experience in SNF/ICF settings (preferred)
• Must be available to work weekends and Hawaii State/Federal holidays as needed
• Willing to arrive by the Thursday before start date for employee health appointment

Knowledge & Skills

• Strong clinical skills in patient evaluation and treatment planning
• Thorough understanding of rehabilitation therapy in SNF/ICF settings
• Accurate and compliant documentation practices
• Effective patient and family education abilities
• Supervisory skills for therapy assistants
• Experience with EMR systems (Net Health Rehab Optima, Epic, Point Click)
