Roles & Responsibilities

• Evaluate and rehabilitate patients with physical functioning disorders and wound care needs
• Develop individualized plans of care following regulatory and clinical practice standards
• Apply evidence-based interventions including manual therapy, therapeutic exercise, and modalities
• Monitor patient progress and adjust treatments to achieve the best outcomes
• Collaborate with families, caregivers, and the care team to support each patient's recovery
• Document care accurately and participate in care conferences and team meetings
• Supervise PTAs, rehab aides, and students as needed

Qualifications

• 1 year of physical therapy experience (new grads welcome with Level II clinical experience)
• LTC or rehabilitation experience required
• Active Massachusetts Physical Therapist license
• Wound care experience is a plus
• A passion for helping others and working as part of a collaborative team

Knowledge & Skills

• Strong patient evaluation and assessment skills
• Clinical documentation and electronic health record proficiency
• Knowledge of therapeutic exercise, modalities, and assistive devices
• Ability to provide patient and family education
• Understanding of discharge planning and regulatory compliance
• Wound care expertise
