Roles & Responsibilities

Provide exceptional physical therapy services to residents in a skilled nursing facility
Develop and implement individualized treatment plans to improve mobility and function
Collaborate with interdisciplinary team to achieve optimal patient outcomes
Document patient progress accurately and timely
Educate patients and families on therapeutic exercises and home programs

Qualifications

Active Physical Therapist license in Wyoming (must be currently active)
Minimum 1 year of experience as a Physical Therapist
Strong communication and interpersonal skills
Ability to work independently and as part of a team

Knowledge & Skills

Knowledge of rehabilitation therapy principles and practices
Skilled in patient assessment and treatment planning
Proficient with EMR systems (training may be provided)
Excellent problem-solving and critical thinking abilities
