Roles & Responsibilities

Provide personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and create tailored care plans
Guide patients and families through exercises, mobility training, and use of therapeutic devices
Collaborate with physicians and care teams to track progress and adjust treatment goals
Support home health aides and caregivers with clear instructions for ongoing rehabilitation
Document clinical notes and progress summaries to keep everyone informed and on track

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy expertise
Patient assessment and rehabilitation planning
Ability to evaluate home environments for safety and mobility
Clear written and verbal communication with patients and care teams
Understanding of Medicare, Medi-Cal, and Title 22 regulations
