Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across your assigned region
Evaluate and treat a diverse caseload including ortho, stroke, joint replacement, and spinal injury patients
Complete OASIS documentation accurately and on time as required by regulations
Collaborate with OT, Speech Therapy, and rehab aides to coordinate comprehensive care
Educate patients and families on home exercise programs and recovery strategies

Qualifications

Active Physical Therapist license in California
At least one year of experience working as a PT, OT, or Speech Therapist
BLS certification required
Experience with OASIS documentation required
Home health setting experience preferred but not mandatory

Knowledge & Skills

Strong understanding of OASIS documentation requirements
Comfortable working independently in home-based care environments
Excellent communication and patient education skills
Ability to manage a varied caseload across multiple diagnoses
Familiarity with Cerner EMR system a plus
