Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $57 per hour

Roles & Responsibilities

Deliver personalized occupational therapy services in patients' homes across your assigned geographic region
Complete OASIS assessments to guide individualized care plans
Collaborate with PT, OT, and Speech colleagues to provide comprehensive home-based rehabilitation
Help patients recover from strokes, joint replacements, spinal injuries, and orthopedic conditions
Document care using Cerner EMR to ensure seamless communication with the care team
Support patients and families with education and adaptive strategies for daily living

Qualifications

Active Occupational Therapist license in California
BLS certification required
At least 1 year of experience as an OT
Experience with OASIS documentation required
Home health experience preferred but not required
Comfortable working independently in home settings

Knowledge & Skills

Strong assessment and care-planning skills using OASIS
Proficient with Cerner EMR documentation
Excellent communication and patient education abilities
Ability to adapt therapy techniques for diverse home environments
Collaborative mindset with PT, OT, and Speech colleagues
