Roles & Responsibilities

• Provide individualized, goal-oriented nursing care to home health patients using the nursing process
• Serve as Case Manager for patients when you are the lead discipline, coordinating comprehensive care plans
• Deliver skilled home health and hospice care to individuals of all backgrounds, ensuring access to high-quality services
• Collaborate with physicians and interdisciplinary team members to implement and adjust care plans
• Educate patients and families on health promotion, long-term care, and disease management
• Document patient assessments, interventions, and outcomes accurately and in a timely manner

Qualifications

• Current, active RN license in Vermont (must be currently active)
• Minimum of 1 year of nursing experience required
• BLS/BCLS certification required
• Home Health experience strongly preferred
• Case management experience is a plus

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Knowledge of the nursing process and individualized care planning
• Skill in case management and care coordination
• Ability to work independently and manage a caseload
• Excellent communication and patient education skills
• Compassionate, community-focused approach to care
