Roles & Responsibilities

• Provide one-on-one physical therapy care to patients in their home setting
• Assess each patient's condition and develop individualized plans of care focused on their unique goals
• Collaborate with an interdisciplinary team and colleagues to ensure every patient's needs are met
• Perform standard therapy measurement techniques, including range of motion, strength, and balance testing
• Utilize a variety of therapeutic modalities to maximize the quality of care provided

Qualifications

• Active Vermont PT licensure is required
• Home health and OASIS experience is a must, as this role includes case management
• McKesson/Netsmart EMR experience is strongly preferred
• Minimum of 1 year of experience required
• Current certifications in ACLS, BLS/BCLS, PALS, NRP, ASHA, CHT, and ACP Training
• Must be available for the full 13-week assignment with no requested time off

Knowledge & Skills

• Expertise in home health and OASIS documentation
• Skilled in patient assessment, care planning, and case management
• Proficiency in ROM, strength, and balance testing
• Knowledge of therapeutic modalities to enhance recovery
• Ability to collaborate effectively with an interdisciplinary team
• Comfort with McKesson/Netsmart EMR systems
