Roles & Responsibilities

Provide compassionate direct patient care and assist with activities of daily living
Monitor and document vital signs, intake/output, and patient observations accurately
Assist nursing staff with treatments, transfers, and safe patient handling
Maintain a clean, safe, and comfortable environment for residents and patients
Communicate clearly and professionally with patients, families, and the care team
Support admissions, discharges, and smooth transitions of care
Participate in ongoing education and maintain required certifications

Qualifications

Current state CNA certification (must be active and in good standing)
CPR/BLS certification (current) or willingness to obtain
Ability to pass background check and drug screen
Comfortable assisting with patient transfers and basic care needs
Reliable, punctual, and a team player who puts patients first
Previous experience in long-term care, skilled nursing, or rehab preferred

Education

High school diploma or GED
Completion of an accredited CNA training program

Knowledge & Skills

Strong patient care and bedside skills
Knowledge of infection control and safe patient handling
Basic charting and documentation skills
Good communication and teamwork
Time management and ability to prioritize tasks
Familiarity with electronic medical records is a plus
