Roles & Responsibilities

• Provide compassionate direct patient care including bathing, dressing, and mobility assistance
• Take and record vital signs accurately and report changes to nursing staff
• Assist with feeding, toileting, and comfort measures while promoting patient dignity
• Maintain clear, timely documentation of care and observations
• Collaborate with nurses and interdisciplinary team members to support individualized care plans
• Ensure a clean, safe environment by following infection control and safety procedures
• Communicate respectfully with patients and families to provide reassurance and support
• Participate in shift handovers to promote continuity of care

Qualifications

• Current CNA certification (required)
• Active/valid certification or registration in Pennsylvania (as applicable)
• Up-to-date immunizations and TB screening per facility policy
• Ability to pass background check and drug screening as required
• Comfort with lifting/transferring patients and performing physical tasks
• Strong communication skills and a team-first attitude
• Previous long-term care or acute care experience is a plus

Education

• High school diploma or GED preferred
• Certified Nursing Assistant (CNA) certification required

Knowledge & Skills

• Basic patient care and ADL assistance
• Vital signs measurement and documentation
• Infection control and standard precautions
• Patient mobility and safe transfer techniques
• Familiarity with electronic charting or willingness to learn
• Strong observational skills and the ability to escalate concerns appropriately
• Compassionate bedside manner and effective teamwork
