Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with activities of daily living
• Support patient safety by performing routine checks and following care plans
• Assist with mobility, transfers and positioning while using safe body mechanics
• Monitor and record vital signs and communicate any changes to the nursing team
• Keep accurate, timely documentation in charts and reports for continuity of care
• Build trusting relationships with residents and their families through clear, respectful communication
• Maintain a clean, infection-controlled environment and follow facility protocols
• Participate in team huddles and care planning to help improve patient outcomes

Qualifications

• Current CNA certification in Pennsylvania or eligibility to obtain state certification
• Basic Life Support (BLS) or CPR certification preferred or willing to obtain
• Demonstrated compassion, reliability and strong teamwork skills
• Comfortable with physical aspects of the role including lifts and transfers
• Prior experience in long-term care, acute care, or similar setting is a plus
• Ability to pass required background checks and health screenings

Education

• High school diploma or GED preferred
• Completion of an approved CNA training program and active CNA credential

Knowledge & Skills

• Knowledge of activities of daily living (ADLs) and resident care basics
• Familiarity with infection control and safety procedures
• Ability to take and record vital signs accurately
• Clear verbal and written communication skills for charting and handoffs
• Teamwork mindset and ability to follow care plans and directions
• Basic charting or electronic health record familiarity preferred
