Roles & Responsibilities

Provide compassionate direct patient care and assistance with activities of daily living
Assist with bathing, dressing, grooming and mobility support in a respectful manner
Take and record vital signs and report changes in patient condition to nursing staff
Maintain a safe, clean and comfortable environment for patients
Accurately document care provided and communicate effectively with the care team
Support admission, transfer and discharge processes as needed

Qualifications

Active CNA certification required
Current basic life support or CPR certification preferred
Ability to pass background check, drug screen and required health screenings
Strong communication skills and a team oriented mindset
Reliable, punctual and committed to patient centered care

Education

High school diploma or GED preferred
Current CNA certification required

Knowledge & Skills

Fundamentals of patient care and safety
Ability to take and record vital signs accurately
Infection control and basic hygiene practices
Clear communication with patients, families and clinical staff
Basic documentation skills and attention to detail
