Roles & Responsibilities

• Provide compassionate care including feeding, mobility support, and assistance with daily living activities
• Follow individualized care plans to align with resident goals, preferences, and safety needs
• Assist with ambulation, transfers, and positioning using gait belts and mechanical lifts
• Support restorative exercises and range-of-motion activities as assigned
• Answer call lights promptly and ensure timely, empathetic responses to resident needs
• Observe and report changes in residents' physical, emotional, or cognitive status to the licensed nurse
• Document ADLs, intake/output, and restorative care accurately in the electronic charting system
• Maintain a clean, safe, and homelike environment for residents
• Follow infection control protocols, including PPE use and hand hygiene
• Float to various units including long-term care, rehab, dementia care, and ventilator unit as needed

Qualifications

• Active BLS and Wisconsin CNA certification at time of submission
• Current certification on the Wisconsin Nurse Aide Registry
• 2+ years of experience as a CNA
• Ability to lift 50+ lbs and safely support residents
• Computer literacy, including electronic medical records

Knowledge & Skills

• Knowledge of infection control standards and precautions
• Understanding of resident rights and safety protocols
• Skill in using mechanical lifts, gait belts, and adaptive equipment
• Strong communication and interpersonal skills
• Ability to work effectively within an interdisciplinary team
