Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in an assisted living setting
• Administer medications and immunizations as required, following facility protocols
• Assess residents’ health status, monitor vital signs, and report changes to the care team
• Document care accurately and promptly in the electronic health record
• Assist residents with activities of daily living and promote independence when possible
• Collaborate with nurses, therapists, and support staff to deliver coordinated care
• Follow infection control procedures and maintain a safe care environment
• Support residents and families with clear, respectful communication and education

Qualifications

• Active Licensed Practical Nurse (LPN) license valid in New York
• Required personal health documentation: COVID-19 and influenza vaccination records and Trinity 10 Panel drug screen
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Comfortable working per diem/contract assignments with flexible availability
• Previous experience in long-term care, assisted living, or similar settings is preferred

Education

• Graduate of an accredited Practical Nursing program; LPN/LVN diploma or certificate required

Knowledge & Skills

• Strong skills in medication administration and basic nursing procedures
• Proficient with resident assessments and monitoring vital signs
• Familiarity with infection prevention and vaccine protocols
• Competent documentation in electronic health records and charting
• Good communication, teamwork, and customer-service orientation toward residents and families
• Ability to prioritize tasks and handle a fast-paced clinical environment
