Roles & Responsibilities

• Plan and conduct treatment programs that help patients restore and improve physical, social, and mental function.
• Complete thorough patient evaluations to maintain quality standards and identify areas that need attention.
• Develop individualized treatment plans using the right clinical techniques and tools for each patient.
• Provide therapy interventions using evidence-based protocols and techniques.
• Create individualized care and discharge plans and communicate them clearly to patients, families, and the care team.
• Document patient progress comprehensively and accurately within established deadlines.
• Supervise assigned assistants and help mentor the next generation of therapy professionals.
• Collaborate as a co-leader in an interdisciplinary approach to patient care.

Qualifications

• At least 2 years of experience as a Physical Therapist
• Active Hawaii state PT license (required)
• At least 1 year of home health physical therapy experience
• Experience with Epic (required)
• Bachelor's degree required; Master's degree preferred
• Travel experience is a plus
• Comfortable meeting a productivity expectation of 25 points per week

Knowledge & Skills

• Expertise in treatment planning and patient evaluation
• Strong clinical documentation skills
• Experience with discharge planning
• Ability to supervise therapy assistants
• Skilled in interdisciplinary collaboration
• Proficient in physical therapy treatment techniques
• Working knowledge of home health care
• Familiarity with Epic EMR
