Roles & Responsibilities

• Coordinate acute care discharge planning for inpatient units, ensuring smooth transitions
• Utilize InterQual and MCG criteria for utilization review and care coordination
• Collaborate with multidisciplinary teams to facilitate timely, safe patient discharges
• Assess patient needs and arrange post-acute services, including home care and rehab
• Document in EPIC with accuracy and maintain compliance with regulatory standards
• Communicate effectively with patients, families, and providers to support care plans

Qualifications

• Active RN license in Nebraska or compact state
• Minimum 2 years of acute care case management experience
• BLS certification required
• Experience with EPIC charting system
• Proficiency in InterQual or MCG criteria
• Strong discharge planning and care coordination skills

Knowledge & Skills

• In-depth knowledge of utilization review and discharge planning processes
• Familiarity with EPIC, InterQual, and MCG/InterQual systems
• Excellent communication and interpersonal skills
• Strong time management and organizational abilities
• Adaptable problem-solving mindset in a fast-paced environment
• Leadership and teamwork capabilities
