Speech Language Pathologist - Skilled Nursing Facility
Location: Pleasant Hill, CA
Start Date: 08-31-2026
Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM
Hours per Week: 35
Bill Rate: $60-$65/hour

Roles & Responsibilities
- Conduct comprehensive assessments of speech, language, cognitive-communication, and swallowing abilities.
- Develop and implement individualized treatment plans to improve patient communication and swallowing function.
- Collaborate with an interdisciplinary team to coordinate patient care and monitor progress.
- Educate patients and families on therapeutic techniques and strategies for continued success.
- Document patient evaluations, progress notes, and discharge summaries accurately and in a timely manner.
Qualifications
- Master's degree in Speech-Language Pathology required.
- Current California Speech-Language Pathologist license in good standing.
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
- Experience in skilled nursing or rehabilitation settings highly desired.
- Strong clinical and documentation skills.
- Ability to work collaboratively with healthcare teams and adapt to patient needs.
Knowledge & Skills
- Knowledge of evidence-based practices in speech and language therapy.
- Understanding of swallowing disorders and modified diets.
- Strong interpersonal and communication skills.
- Proficiency in electronic health records.
- Compassionate and patient-centered approach.
- Ability to develop and implement individualized treatment plans.
- Excellent time management and organizational skills.
- Knowledge of regulatory standards in skilled nursing facilities.
