Roles & Responsibilities

• Maintain and run a comprehensive diabetes education program that coordinates clinical providers, therapists, mental health staff, and health educators
• Provide direct diabetes education and case management to people with diabetes, prediabetes, and gestational diabetes
• Maintain and manage diabetes registries and ensure patients receive recommended annual exams, labs, immunizations, and education
• Coordinate primary and secondary prevention goals with the SEARHC Consortium Diabetes team
• Lead and coordinate the annual IHS Diabetes Audit and use audit/RPMS data to drive quality improvements
• Train and educate medical providers, dietitians, nurses, and staff on IHS Standards of Care for Adults with Type 2 Diabetes
• Monitor and support patient self-management tools and equipment to reduce chronic care costs
• Serve as a helpful resource to staff and patients—answer inquiries, relay messages professionally, and model SEARHC standards

Qualifications

• Valid, current Registered Nurse (RN) license in the State of Alaska (required)
• Certification as a Diabetes Educator OR eligibility to apply for certification within 2 years
• Clinical competency assessment required within 3 months of hire and every three years
• Preferred: 3 years general nursing or dietetic experience
• Preferred: 1 year work experience in diabetic care and management
• Ability to lift/move up to 50 lbs and perform standard physical job duties
• Demonstrated cultural competence and ability to work with Alaska Native populations
• Strong oral and written communication, organization, and teaching skills

Education

• Active RN license (State of Alaska)
• Certification as Diabetes Educator OR eligibility to pursue certification within 2 years
• Clinical Competency Assessment per employer requirements

Knowledge & Skills

• Diabetes case management principles, practices, and procedures
• Familiarity with IHS Standards of Care for Adults with Type 2 Diabetes
• Experience with diabetes registries, RPMS tools, and quality audits
• Patient education and health promotion techniques
• Comfort leading group programs and training clinical staff
• Proficiency with routine computer use, documentation, and reporting
• Good judgment, prioritization, and ability to work independently amid interruptions
