Roles & Responsibilities

Assess residents’ functional abilities and develop individualized physical therapy plans
Deliver skilled therapeutic interventions to improve mobility, strength, and independence
Collaborate with nursing and interdisciplinary team members to coordinate care
Document progress and plan updates clearly and timely in the resident record
Educate residents and families on home exercises, safety strategies, and discharge planning
Participate in team meetings and quality improvement efforts to elevate patient outcomes

Qualifications

Licensed Physical Therapist in Colorado or eligible for Colorado licensure
DPT or equivalent Physical Therapy degree (new grads encouraged to apply)
Comfort working in a Skilled Nursing Facility (SNF) environment
Strong communication and teamwork skills to coordinate with interdisciplinary staff
Ability to pass background check and any facility-required pre-employment screenings

Education

Doctor of Physical Therapy (DPT) or equivalent Physical Therapy degree (open to new graduates)

Knowledge & Skills

Knowledge of SNF care, geriatrics, and rehab best practices
Skilled in assessment, treatment planning, and therapeutic exercise
Solid clinical documentation and time-management abilities
Ability to educate residents and caregivers in an approachable, effective way
Comfort working within facility policies and collaborating with nursing staff
