Roles & Responsibilities

• Provide comprehensive physical therapy assessments and individualized treatment plans for residents in a skilled nursing facility
• Implement therapeutic interventions to improve mobility, strength, and balance
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Document patient progress accurately and maintain compliance with facility and regulatory standards
• Educate patients and families on home exercise programs and fall prevention strategies
• Adapt treatment approaches to meet the unique needs of each patient

Qualifications

• Active Physical Therapist (PT) license in Virginia
• Doctor of Physical Therapy (DPT) degree
• New graduates are welcome to apply
• SNF or rehabilitation experience preferred, but not required

Education

• Doctor of Physical Therapy (DPT) degree

Knowledge & Skills

• Expertise in physical therapy assessment and treatment planning
• Knowledge of SNF rehabilitation practices and regulatory requirements
• Strong clinical reasoning and problem-solving skills
• Excellent communication and interpersonal skills
• Proficiency with electronic medical records and documentation
• Ability to work collaboratively in a multidisciplinary team
