Roles & Responsibilities

• Evaluate and assess patients' physical therapy needs in a skilled nursing and rehabilitation setting
• Develop and implement individualized treatment plans to improve mobility, strength, and function
• Provide therapeutic interventions and exercises to help patients recover from illness or injury
• Collaborate with nurses, physicians, and other healthcare professionals to ensure comprehensive patient care
• Document patient progress and adjust treatment plans as needed to achieve optimal outcomes
• Educate patients and their families on treatment strategies and home exercise programs

Qualifications

• Active Physical Therapist (PT) license in New Hampshire
• Graduate of an accredited Physical Therapy program
• New graduates are encouraged to apply
• Experience in skilled nursing, long-term care, or rehabilitation settings is a plus
• Strong communication and interpersonal skills
• Ability to work Monday through Friday with occasional weekend rotation

Knowledge & Skills

• Knowledge of rehabilitation techniques and therapeutic exercise
• Patient assessment and care planning skills
• Ability to document accurately and timely
• Familiarity with electronic medical records (EMR) systems
• Compassionate, patient-centered approach to care
• Time management and organizational skills
• Collaborative teamwork skills
