Roles & Responsibilities

• Evaluate patients' physical and functional status to develop individualized treatment plans
• Implement therapeutic exercises and modalities to improve mobility, strength, and balance
• Collaborate with an interdisciplinary care team to ensure holistic patient progress
• Educate patients and families on home exercise programs and injury prevention
• Document patient progress accurately and adjust treatment plans as needed
• Advocate for patients' goals and celebrate their milestones along the way

Qualifications

• Active Physical Therapist license in California (or eligibility to obtain)
• Graduate of an accredited Physical Therapy program
• Previous experience in skilled nursing or rehabilitation settings is a plus
• Excellent communication and interpersonal skills
• Compassionate, patient-centered approach to care

Knowledge & Skills

• Strong knowledge of therapeutic exercises, modalities, and manual therapy techniques
• Ability to assess functional limitations and develop evidence-based treatment plans
• Proficiency with electronic documentation and medical records
• Team collaboration and interdisciplinary communication
• Time management and ability to prioritize patient caseload
