Roles & Responsibilities

• Evaluate patients' physical abilities and develop individualized treatment plans
• Provide hands-on therapy, exercises, and modalities to improve mobility and reduce pain
• Educate patients and families on home exercise programs and safety strategies
• Collaborate with physicians, nurses, and other therapists to coordinate care
• Document patient progress accurately and timely in electronic medical records
• Participate in discharge planning and recommend adaptive equipment as needed

Qualifications

• Active California Physical Therapist license
• Doctor of Physical Therapy (DPT) degree or equivalent
• Current BLS certification
• Experience in post-acute or skilled nursing setting preferred
• New grads with strong clinical skills are welcome to apply
• Compassionate, patient-centered approach
• Ability to work effectively as part of a team

• Strong clinical assessment and manual therapy skills
• Excellent communication and interpersonal abilities
• Ability to motivate and empower patients
• Proficiency with electronic medical records
