Roles & Responsibilities

  • Evaluate patients' physical abilities and develop individualized treatment plans to restore function and mobility.
  • Implement therapeutic exercises, manual therapy, and modalities to improve strength, balance, and coordination.
  • Collaborate with nurses, physicians, and other therapists to ensure comprehensive, patient-centered care.
  • Educate patients and their families on home exercise programs and injury prevention strategies.
  • Monitor patient progress, document outcomes, and adjust treatment plans to achieve optimal recovery.
  • Advocate for patients' wellness and independence in a supportive post-acute environment.

Qualifications

  • Active California Physical Therapist license (or eligibility to obtain one)
  • Graduation from an accredited Doctor of Physical Therapy (DPT) program
  • CPR/BLS certification from the American Heart Association
  • Experience in skilled nursing, rehab, or post-acute care setting (preferred)
  • Strong communication and interpersonal skills
  • Compassionate, patient-centered approach to care

Knowledge & Skills

  • Expertise in patient assessment and functional mobility evaluation
  • Proficiency in therapeutic exercise prescription and manual therapy techniques
  • Knowledge of geriatric rehab and post-acute care best practices
  • Skill in using electronic health records for documentation
  • Ability to work collaboratively within a multidisciplinary team
  • Excellent problem-solving and critical-thinking abilities
