Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM
Hours per Week: 35
Bill Rate: $56-$61/hour

Roles & Responsibilities
- Evaluate patients' physical abilities and develop individualized treatment plans to restore function and mobility.
- Implement therapeutic exercises, manual therapy, and modalities to improve strength, balance, and coordination.
- Collaborate with nurses, physicians, and other therapists to ensure comprehensive, patient-centered care.
- Educate patients and their families on home exercise programs and injury prevention strategies.
- Monitor patient progress, document outcomes, and adjust treatment plans to achieve optimal recovery.
- Advocate for patients' wellness and independence in a supportive post-acute environment.
Qualifications
- Active California Physical Therapist license (or eligibility to obtain one)
- Graduation from an accredited Doctor of Physical Therapy (DPT) program
- CPR/BLS certification from the American Heart Association
- Experience in skilled nursing, rehab, or post-acute care setting (preferred)
- Strong communication and interpersonal skills
- Compassionate, patient-centered approach to care
Knowledge & Skills
- Expertise in patient assessment and functional mobility evaluation
- Proficiency in therapeutic exercise prescription and manual therapy techniques
- Knowledge of geriatric rehab and post-acute care best practices
- Skill in using electronic health records for documentation
- Ability to work collaboratively within a multidisciplinary team
- Excellent problem-solving and critical-thinking abilities
