Roles & Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and create individualized treatment plans
• Provide hands-on physical therapy interventions to improve strength, mobility, and functional independence
• Educate patients and families on therapeutic exercises, safe movement, and home modifications to support recovery
• Collaborate with occupational therapists, nurses, physicians, and other team members to coordinate care
• Monitor patient progress, adjust treatment plans as needed, and drive measurable outcomes
• Maintain accurate, timely documentation of assessments, progress notes, and care plans in line with facility policy
• Ensure care and documentation meet regulatory requirements and best practices in physical therapy

Qualifications

• Valid Physical Therapy license in California (active and unrestricted)
• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Experience in a skilled nursing facility or similar rehab setting preferred
• Strong communication and interpersonal skills for patient and team interactions
• Demonstrated problem-solving ability and a compassionate, patient-centered approach
• Comfortable working independently and as part of an interdisciplinary care team

Education

• BS, MS, or Doctoral degree (DPT) in Physical Therapy from an accredited program

Knowledge & Skills

• Patient evaluation and treatment planning
• Therapeutic interventions and therapeutic modalities
• Use of adaptive equipment and recommendations for home modifications
• Accurate clinical documentation and regulatory compliance
• Interdisciplinary collaboration and care coordination
• Patient and family education techniques
