Roles & Responsibilities

Evaluate and treat patients with movement dysfunction and pain to restore function and independence
Design and implement therapeutic interventions to reduce fall risk and improve mobility
Develop and manage discharge plans to support safe transitions from care to home or next-level services
Document evaluations, progress notes, daily notes, discharges, and insurance communications thoroughly and timely
Coordinate care through phone communication with physicians, insurance companies, patients, and interdisciplinary team members
Support the clinical environment by assisting with treatment room cleaning, laundry, and maintaining a safe workspace
Perform other duties as requested to contribute to a smooth, patient-centered clinic experience

Qualifications

Licensed Physical Therapist in Wisconsin or eligible for licensure
Strong patient assessment and documentation skills
Comfortable managing insurance authorization/approval processes and related communications
Excellent verbal communication for coordinating with patients, families, and interdisciplinary teams
Compassionate, detail-oriented, and able to handle light cleaning/room prep tasks as part of patient care

Education

Graduation from an accredited Physical Therapy program (DPT preferred)
Current state PT license (or eligibility) required

Knowledge & Skills

Patient assessment and evaluation
Therapeutic interventions and mobility training
Pain management strategies
Fall risk reduction techniques
Discharge planning and care coordination
Accurate clinical documentation and progress notes
Insurance authorization workflows and phone-based communication
