Roles & Responsibilities

Deliver personalized physical therapy treatments that help residents regain strength and independence
Collaborate with an interdisciplinary team to create meaningful care plans
Assess patient progress and adjust therapy techniques to maximize outcomes
Educate patients and families on safe exercise routines and home modifications
Document treatment sessions accurately to support continuity of care

Qualifications

Active Physical Therapist license in Tennessee
Doctor of Physical Therapy (DPT) degree or equivalent
New graduates are welcome to apply
Strong communication and interpersonal skills

Education

Doctor of Physical Therapy (DPT) or Master's degree in Physical Therapy from an accredited program

Knowledge & Skills

Skilled in geriatric rehabilitation and fall prevention
Comfortable working in a Skilled Nursing Facility setting
Ability to adapt therapy plans for diverse patient needs
Team-oriented mindset with a passion for patient-centered care
